Fraud Report Form
The Council's standard Fraud Report Form is available to download at the bottom of this page. It is a Microsoft Word file that you should be able to complete using your computer.
Once you have filled in as much information as you can you can either:
- print off the form and send it through the post, or
- save the file to a convenient location on your computer and then send it to us electronically by uploading it as shown below.
Whichever method you choose, your information will be treated in the strictest confidence. We take all allegations of fraud seriously and, although we can't give you any feedback on the information you send us, you can be confident that your concerns will be addressed.
