If you are currently living in supported accommodation then you may be eligible to access accommodation under the Move-on Protocol. Ask your support worker for more details. You and your support worker will first need to complete a MOP form, and then a R2M form when you are ready to move-on to your own accommodation. The forms are available to download below.
