What is the Health Referral Scheme?
The Health Referral Scheme is a partnership between North East Derbyshire District Council and the Derbyshire County Primary Care Trust.

What are the aims of the Scheme?
- To improve the standard of health for individuals who have been recommended by a member of the Primary Health Care Team as being suitable for developing their fitness/activity levels and general well being through appropriate regular exercise.
- To encourage people to take greater responsibility in improving their own health by pursuing a more physically active life style.
How do I get on the Scheme?
Who can refer me to the Scheme?
Any Health Practitioner at participating surgeries such as a GP, Health Visitor, Practice Nurse or District Nurse. Please see the list of Surgeries and Specialists (click the link on the left).
Do I need a reason to be referred?
There are criteria for you to be referred to the Scheme, these are:
- Weight reduction for a body mass index 30 plus
- Stroke (CVA)
- High Blood Pressure
- Diabetes (type 1 & 2)
- Chronic Obstructive Pulmonary Disease (COPD) / Asthma
- Osteoarthritis
- Osteoporosis
- Coronary Heart Disease (CHD)
- Social / Mental Health / Depression
- Mobility Problems
(The above reasons are under the discretion of the referring practitioner)
How long does the Scheme last?
The Scheme is over a 10 week period. You will see an exercise officer at the start to discuss your physical activity likes and dislikes and jointly agree a realistic and achievable activity plan. Any medical conditions are discussed and appropriate forms completed. A development record is issued for you to record your activity levels. You will be given a programme of activity leaflet where you can attend group sessions and supported supervised classes taken by staff at leisure facilities or attend any of the individual activities on offer.
What happens when the Scheme ends?
At the end of the 10 weeks, any changes in activity levels, body measurements, confidence, motivation, will be recorded. You can discuss what to do next with the Exercise Officer/Instructor and will be encouraged to keep on going on any activities you were doing throughout the Scheme . A brief report will be submitted to your Surgery to inform them of completion and progress.
What are the costs for the Scheme?
The first 5 visits to use either the swimming pool, gym or organised Health Referral session are free. From the 6th session onwards you will be charged a reduced fee based on individual circumstances. This is done by applying for either a Leisure Membership if you are in receipt of Housing or Council Tax benefit or a Concessionary Membership if you are not in receipt of the above benefits, as part of your Access2Leisure Card.
The Council's Leisure facilities offer a large variety of activities. Please click on the links on the left to see a programme of activities that may be appropriate for people on the Health Referral Scheme or anyone wanting more gentle exercise sessions.
