Practices Participating in the Health Referral Scheme
This page shows details for all surgeries and health specialists that are part of our Health Referral Scheme.
Select the location of your health centre from the drop-down box or browse through the list by clicking 'Show All'.
You can also search for your practice by entering part of the name or address in the Keyword box (please note though that surgeries are not list under the GPs name).
If your surgery or health centre is not listed, it may be that they have not yet signed up to the scheme. Please contact us for more information or ask at your local surgery.
