Health Referral Scheme Information
The council works in partnership with Derbyshire County NHS and Killamarsh Parish Council to provide a Health Referral Programme.
The Health Referral scheme aims to increase exercise and activity in everyday life for people who want to improve their health.
It is especially for those people who currently do either no or very little physical activity but who are motivated to do more. Being active each day makes us feel good, boost our confidence and improves the health & wellbeing of everyone in North East Derbyshire.
The Derbyshire Community Health ServicesWaistwiseteam - which aims to target weight loss for men and women - is integrated with the Health Referral service. This extended service will enable us to offer improved nutritional and dietary advice and support.

Frequently Asked Questions (FAQ)
To have access to the scheme you must visit your surgery and ask a member of the Health Care Team to refer you using the appropriate referral form.
A health professional can refer you to your nearest Health Referral Scheme. All you need to do it make an appointment with your GP who will decide with you whether you can be referred to Health Referral.
You may see other health professionals such as practice nurses, weight management advisors, stop smoking advisors, community mental health staff, hospital staff – and they can also refer you to Health Referral.

Health Referral is for adults (16 years and over) who:

Take part in little or no exercise.

Are willing and motivated to become more active to benefit their health in the long term.

Are living with health conditions which exercise would help: - weight loss, heart problems, high blood pressure, diabetes, mobility problems (joint, muscle pain) or you live with a level of mental ill-health.

The Health Referral administration team will contact you to invite you to a 1:1 appointment to talk about your referral and plan an exercise programme that is best for you that will be suited to your personal needs and interests with realistic goals agreed. Activities can include:
- Gym
- Zumba
- Swimming (Swimfit)
- Pilates
- Aquacise
- Chair based exercise
- Group exercises
- Walking for Health
- Jog Derbyshire
- Low impact circuit training
- Cardiac rehabilitation
Participating venues and suitable exercise sessions include:
A development record will be issued to you so you can record your activity levels. You will also be given an activity leaflet which will provide all the essential information about where you can attend group sessions, supervised classes and any of the individual activities on offer.
The scheme will last for a 12 week period. Your progress and goals will be assessed after this period and a personal ongoing exercise regime will be planned to suit your requirements to maintain your new active lifestyle.
Wear comfortable, loose clothing so that you can move around easily. Your shoes should be flat with a soft sole and comfortable. There is no need to buy special clothes.
Health Referral is offered at a significantly discounted rate to support all, including vulnerable and disadvantaged people.
People who are in receipt of Council Housing Benefit/Council Tax Benefit will pay £1.95 per session for the gym, swim and low or high impact circuits. People not in receipt of any benefits will be charged £2.70 for the same sessions.

Classes that are not led by Health Referral Instructors are charged at the full rate (e.g Aquacise, Zumba, and Jog Derbyshire).
These prices are for Dronfield Sports Centre, Eckington Swimming Pool and Sharley Park Leisure Centre in Clay Cross.
Killamarsh Sports Centre is run by the Killamarsh Parish Council and prices may vary.
If you are planning to attend the centre for physical activity 2 or more times per week then it would be beneficial to purchase a Super Saver for £18.50 per month.
You can use the gym, swimming and low impact circuit training facilities as many times as you like in a month. (Pass runs out one month from day of purchase and can be renewed as required).
